Background Information
the "background" tab found under the patient profile is where you can enter important background information about the patient this information includes the family history, birth and development history, health history, social history, educational history, previous testing results, and classroom observations users can add additional backround sections based on their report writing needs important reminder if a user leaves a background section blank, it will not show up in the report output so if a particular section is not relevant to an examinee, leave it blank to make this process easier, we have provided a "checklist" feature for some sections of the background information this allows you to interact with a pre determined checklist and select relevant information, which will automatically populate the text field this feature can save you time and ensure that you are including all necessary information to use the checklist, simply click on "start checklist" button next to the narrative you want to fill out and select the relevant items from the checklist provided the checklist will prompt you to provide additional details as necessary once you have completed the checklist, the information will automatically populate the corresponding text field in addition to the checklist, you can also manually enter any additional background information in the text fields provided this will allow you to provide a comprehensive overview of the patient's background, which can be crucial in providing accurate diagnoses and treatment recommendations background ( sample data is shown in the image below ) example checklist (health history) adding a new background section to add a new background narrative section to the "background" portion of the report, start by clicking on settings on the left side menu options scroll down the page to find the custom background field definitions option which looks like this follow the prompt to add a custom field, or in this case a name for a new background narrative section for example, aftering clicking on the add custom field button, the screen looks like this as an example, "mental status exam" could be typed in (see below) make sure to click on the save button after the label for the new background information section has been typed in if you now click on view patients , select a patient, choose the background tab at the top of the screen, and scroll down to the botom of the page, you will see the new narrative section for mental status exam all of the same editing and ai functions will be available to the user in the mental status exam narrative section as in all other background sections once a new backgound narrative section is created, it will be visible for all patients as a choice remember if no narrative is entered ih any background section, it will not appear in the report output users can add any number of additional background narrative sections based on the type of report being written possible additional sections could include, but are not limited to records reviewed relevant history transition services occupational history collateral information review mental status examination mental health history criminal and legal history criminal allegations competency to stand trial substance abuse history military history limits of confidentiality credentials relevant to this evaluation
