Once the rater submits the NPCC-3 / NPCC-4 form you can insert their comments into the report by doing the following:

1. Go to the [Report Writer](docId\:K9jpJxfA1fNdtHBpgk8aE)  tab of a given patient.

2. Navigate to any NPCC-3 / NPCC-4 table. Beneath it you will see a text box to enter in your narrative.

3. For ease of use, you can easily insert rater comments from the NPCC-3 / NPCC-4 for a given section directly into your narrative simply by pressing the "Add from Survyes" dropdown found beneath the narrative box.

4. Once you click a given comment to add it will automatically be addded to your narrative for that NPCC-3 / NPCC-4 section.
